Evergreen Church Planter Assessment
Welcome to the Evergreen Church Planter Assessment process. You have arrived at this unpublished page because you have been invited by Evergreen Network, or a sister organization, to participate in one of our Assessment Events held throughout the year. Before you are invited to the Assessment Event, we have a Pre-Assessment process we need both you, and your spouse separately, to complete. Pre-Assessment
The first step is for you, and your spouse separately, to complete the questionnaire below before moving on to the inventories and psychologist interview. Please complete the questionnaire and all inventories by January 4th.
Questionnaire
This four-part questionnaire will take some time and thought to complete, so please start this process when you have a couple of hours to complete it. If you need to save and continue later, please select that option at the bottom of the form, type your email, and it will email you a link to continue on later. Additionally, we strongly recommend copying your answers onto another document as a back-up.

Inventories
Now that you have completed the above questionnaire, you can move on to complete each of the following inventories. You will receive an email with further instructions and invitations to complete the first four assessments listed. The ACE Questionnaire and Background Check can be completed through this page. When finished, please upload to each submission page below.

- This should be completed through the link and does not require you to upload anything.
Psychologist Interview
The final step in the Pre-Assessment process is for you and your spouse together to schedule an interview with a psychologist. We will make that introduction via email so you can set up your appointment. You must participate in this meeting between January 4-23, so we highly encourage scheduling it as early as possible.
Assessment Event
Upon the successful completion of the Pre-Assessment process above, you will be considered as a candidate for our next Assessment Event. This 4-day event is designed to provide you an opportunity to explore your competencies and skills for establishing a new church under the counsel of experienced and trusted leaders. This will be an invitation-only event.
We do not provide a schedule for these events, other than location, start and end times, and lodging details. We actually do not want you to attempt to prepare, we just want you to show up and be yourself.
During the event, you will learn a lot about your strengths and growth opportunities and be provided with a formal recommendation for next steps in your journey.
